Gastric Sleeve ( Vertical Gastrectomy)

What is Gastric Sleeve?

Vertical Gastrectomy ,  also commonly known as Gastric Sleeve surgery, is one of the most effective treatments for obesity and weight-related health problems. Over the past decades, obesity has become increasingly common, reducing both quality of life and life expectancy.

When lifestyle changes such as diet and exercise are no longer enough, bariatric surgery may be recommended. Among the different options, Gastric Sleeve is currently the most popular weight loss surgery worldwide. It allows patients to lose weight safely, return to daily routines quickly, and achieve long-term results.

This procedure is performed laparoscopically (through tiny incisions rather than open surgery). Thanks to this minimally invasive approach, complication rates are very low — in fact, studies show it is even safer than gallbladder removal.

Health Benefits of Gastric Sleeve

Beyond significant weight loss, Sleeve Gastrectomy can improve or even resolve many obesity-related health issues, including:

  • Type 2 Diabetes

  • High blood pressure (hypertension)

  • High cholesterol

  • Sleep apnea

  • Joint pain and mobility problems

In women, Gastric Sleeve has also shown positive effects on Polycystic Ovary Syndrome (PCOS), helping regulate hormones and even improving fertility and pregnancy outcomes.

Who Is Eligible for Sleeve Gastrectomy Surgery?

Not every patient is automatically a candidate for Gastric Sleeve surgery. The decision is always made after a detailed evaluation by our specialist bariatric team, taking into account health history, test results, and overall suitability for surgery.

Below are some of the main criteria considered when assessing eligibility for Sleeve Gastrectomy:

  • Body Mass Index (BMI): A BMI of 30 or higher is generally required, especially if combined with an obesity-related health condition.

  • COVID-19 status: Patients must not have tested positive for COVID-19 within the month prior to surgery.

  • Infections: Active infections must be resolved before surgery can be safely performed.

  • Acid reflux and hiatal hernia: Patients with reflux caused by a hiatal hernia may not be suitable candidates.

  • Chronic conditions: Health issues such as thyroid disorders, Crohn’s disease, or diabetes may require additional testing before surgery.

  • Surgical history: Previous operations can sometimes affect eligibility and need to be reviewed by the surgical team.

  • Medical clearance: In some cases, a letter of approval from the patient’s personal doctor may be required, or further consultations may take place once in Turkey.

Next Step: Personalized Evaluation

Every patient is unique, and only after a comprehensive medical assessment can our team confirm if Gastric Sleeve is the right option for you. To find out whether you qualify, we recommend speaking directly with a MedFinders Patient Coordinator, who will guide you step by step through the process.

What Should You Do Before Gastric Sleeve Surgery?

Once you have been approved for Sleeve Gastrectomy, your Patient Coordinator will provide a personalized treatment plan. This usually includes a liver-shrinking diet to follow before admission, which helps prepare the body and ensures a safer operation.

In addition, patients are required to follow some important pre-surgery rules and lifestyle adjustments to minimize risks and improve recovery.

Key Pre-Surgery Guidelines

  • Fasting: Do not eat or drink anything in the 12 hours prior to surgery. An empty stomach is essential for anesthesia safety.

  • Vitamin K: Stop taking Vitamin K and other anticoagulants 30 days before surgery, as these can thin the blood and increase the risk of internal bleeding.

  • Hormonal contraceptives: Oral contraceptives and hormone-based methods (such as coils, HRT patches, and implants) should be discontinued 30 days before surgery, as they raise the risk of blood clots. They may be restarted one month after surgery. Non-hormonal options are considered safer for patients undergoing bariatric surgery.

  • Smoking: Avoid smoking from one week before surgery until at least three weeks afterward, as smoking reduces blood flow and may increase the risk of complications, including leakage.

  • Alcohol: Ideally, alcohol should be avoided for three months before surgery. It is especially important not to drink alcohol during the week before surgery, as it increases liver strain and can complicate anesthesia.

Preparing for Success

These pre-operative steps are critical for reducing complications and ensuring a smooth recovery. By following the liver-reducing diet and respecting the medical instructions given by your care team, you greatly increase your chances of a safe surgery and lasting results.

How Is Sleeve Gastrectomy Performed?

Gastric Sleeve surgery is carried out under general anesthesia, meaning patients are fully asleep and feel no pain during the procedure. The operation is performed laparoscopically, a minimally invasive method that uses 4–5 very small incisions in the abdomen. This technique allows for faster healing, less discomfort, and a lower risk of complications compared to traditional open surgery.

During the operation, the surgeon inserts a tiny camera (laparoscope) and fine surgical instruments through these keyhole incisions. About 80% of the stomach is carefully removed, including the fundus region, which is responsible for producing most of the hunger hormone ghrelin. By reducing this area, appetite naturally decreases.

The remaining stomach is reshaped into a slender, banana-like sleeve, which restricts the amount of food a patient can eat while still allowing normal digestion. The stomach is then securely sealed with surgical staplers, and the removed portion is extracted through one of the incisions. The incision sites are finally closed with strong sutures to minimize the risk of hernia.

This precise and minimally invasive technique makes Sleeve Gastrectomy one of the safest and most effective bariatric procedures worldwide.

  • Blood thinners – to reduce the risk of blood clots

  • Stomach protectors – to support healing and reduce irritation

  • Antibiotics and pain relief – prescribed if needed for comfort and safety

Post-Surgery Diet Stages

Your diet after surgery progresses step by step to allow safe healing:

  1. Liquid Stage (2 Weeks )
Only liquids such as protein shakes, broths, herbal teas, and soups are allowed.

  2. Puree/Bland Stage (3 Weeks )
Soft, blended foods and purees are introduced gradually.

  3. Solid Foods (After 6th Week)
Patients can start eating solid meals again, but liquids and solids should not be consumed together.

Additional dietary restrictions:

  • Avoid caffeinated and carbonated drinks for at least 2 months.

  • Decaffeinated coffee can be introduced safely after 10 days.

  • Smoking, alcohol, and recreational drugs must be avoided for at least 3 weeks post-surgery.

Physical Activity After Surgery

  • Light walking is encouraged as soon as possible.

  • Avoid carrying anything heavier than 3 kg (6.6 lbs) for the first weeks.

  • Patients can usually return to non-strenuous work within 2 weeks.

  • Sexual activity and heavy physical exertion should be avoided for 1 month.

  • Exercise to support weight loss can be started after 2 months.

Healing & Stitches

The small incisions are closed with dissolvable stitches, which usually heal naturally. If stitches do not dissolve on their own, a visit to your GP may be needed for removal.
